The fundamental difference
An eyebrow transplant implants real, living hair follicles that grow permanently. Microblading deposits semi-permanent pigment that fades. These are not two versions of the same solution — they produce categorically different results for categorically different patients. Understanding which is right for you starts with understanding what each actually does.

How eyebrow transplant works
Eyebrow transplant uses the FUE (follicular unit extraction) technique — the same approach used for scalp hair restoration. Individual hair follicles are harvested one by one from a donor area (typically the back of the scalp, behind the ear, or from body hair depending on the texture desired) using a small punch device. The follicles are then implanted individually into the brow area at the correct angle and direction to mimic natural brow hair growth patterns.
The implanted follicles retain their genetic identity from the donor site — they are scalp follicles that happen to be growing in the brow. This means they grow continuously like scalp hair, not at the slower rate of natural brow hair. Patients who have had eyebrow transplants need to trim their brows regularly — typically every 2–4 weeks — to maintain the desired length. This is the primary maintenance consideration that candidates need to understand before proceeding.
At Plump Medical Spa in Newport Beach, eyebrow transplant is $5,800 flat for both brows — performed personally by Dr. Mortazavi. The flat fee structure means cost does not increase with the number of grafts needed within the standard eyebrow restoration range.
The eyebrow transplant timeline
Wk 1
Immediate post-procedure
Small scabs form around each implanted follicle. Mild redness and swelling in the brow area. Visible but manageable. Most patients feel comfortable in public by day 5–7.
Wk 2–4
Shedding phase — do not panic
Most transplanted hairs shed within 2–4 weeks. This is completely normal and expected — it is the telogen phase of the hair cycle. The follicle itself remains alive beneath the skin. Most patients look similar to their pre-procedure state during this window.
Mo 3–4
New growth begins
Transplanted follicles enter the active growth phase. Fine, new brow hairs begin emerging. Growth is initially sparse and may appear patchy. This is the beginning of the result — not the final outcome.
Mo 6–12
Progressive filling — result develops
Hair density increases progressively. Brow shape becomes visible and defined. Trimming begins as the transplanted hairs grow to brow length. Most patients see their clearest result between months 8 and 12.
Year 1+
Full result — permanent maintenance
Full result is assessed at 12 months. The brow continues growing permanently from this point. Regular trimming every 2–4 weeks is the only ongoing maintenance. No top-up appointments, no fading.
How microblading works
Microblading uses a handheld tool with a fine blade to make shallow cuts in the skin and deposit pigment into the incisions, creating hair-stroke impressions. The result looks like individual brow hairs drawn on the skin. A numbing cream is applied before the procedure, and the session typically takes 2–3 hours including the initial design consultation.
The pigment sits in the superficial dermis — deeper than temporary makeup but shallower than a traditional tattoo. It fades over 1–3 years depending on skin type, sun exposure, skincare, and the original pigment used. Annual touch-ups are typically required to maintain the result. Patients with oily skin, large pores, or certain skin conditions may find pigment retention is poor — the pigment can blur, fade unevenly, or develop a color shift (commonly toward red or gray) over time.
The most important limitation of microblading
Microblading creates the appearance of brow hair — it does not create hair. For patients with significant hair loss, very sparse brows, or absent brows due to medical conditions, microblading provides a cosmetic impression but does not address the underlying absence of follicles. No amount of microblading creates actual hair growth. Only an eyebrow transplant restores real hair where follicles are absent or significantly depleted.
Who each treatment is right for
Eyebrow transplant — right for you if...
Your brows are significantly thinned, sparse, or largely absent
You have lost brow hair from over-plucking, alopecia, chemotherapy, or scarring
You want a permanent solution — no fading, no touch-ups
You are comfortable with regular trimming as ongoing maintenance
You want natural hair texture and movement, not a drawn-on look
You have oily skin or large pores that cause poor microblading retention
Microblading — right for you if...
You have existing brow hair but want more definition, shape, or fullness
You are not ready for a permanent commitment and want to try the look first
Your budget does not allow for a transplant at this time
You want a result visible immediately after healing, not after 12 months
You have dry or normal skin (better pigment retention)
You are comfortable with annual touch-ups to maintain the color
Can you do both? Yes — and some patients do. Microblading provides an immediate result while waiting for transplant hair to grow in (the transplant shedding and regrowth phase takes 6–12 months). The two are not mutually exclusive, though the microblading should not be applied directly over transplanted follicle sites. This should be discussed at consultation before proceeding with either treatment.
Frequently asked questions
Is an eyebrow transplant permanent?
Yes. Eyebrow transplant using the FUE technique moves permanent hair follicles from the donor area to the brow, where they grow for life. The transplanted hair grows continuously and requires regular trimming every 2–4 weeks. The result does not fade or require top-ups.
Does microblading work on sparse or absent brows?
Microblading creates a pigment impression of brow hair strokes — it does not create actual hair. For patients with very sparse brows it can improve appearance, but for patients with largely absent brows, the impression looks less natural than real hair. Only a transplant restores actual follicles where hair is absent.
How long is recovery after eyebrow transplant?
Visible redness and small scabs resolve within 7–10 days. Transplanted hairs shed at weeks 2–4 (this is normal). New growth begins at 3–4 months. The clearest result is visible at 8–12 months. Most patients are comfortable in social settings within 1–2 weeks of the procedure.
