The triceps makes up the majority of upper arm mass. When well-developed, it adds width and definition to the back of the arm — desirable for some patients, and the opposite of what others want, particularly in sleeveless clothing. Partially relaxing the muscle reduces that bulk over several weeks.

This is a newer application, and worth understanding as one.

Neurotoxin for muscle slimming is well established in the masseter and increasingly in the trapezius. Use in the triceps is off-label and not supported by a substantial published literature. Dr. Mortazavi performs it based on clinical experience rather than established protocol, and doses conservatively for that reason.

Off-label use of an approved medication is legal and routine in medicine. But there's a difference between well-mapped territory and newer ground, and you should know which one you're in. Any practice presenting arm slimming as a standard, well-studied treatment is overstating what's known.

The functional consideration. The triceps extends the elbow — it's what straightens your arm, and it does the work in pushing, pressing, and lowering anything under control. Weakening it is more noticeable in daily activity than relaxing a jaw or shoulder muscle.

Treatment is approached as partial softening rather than substantial weakening: conservative initial dosing, with adjustment at follow-up if warranted. Temporary reduction in pushing strength or fatigue with repetitive arm extension is possible and resolves as the effect wears off.

Bulk isn't the only reason arms look heavy. Upper arm appearance is a combination of muscle, subcutaneous fat, and skin laxity. Neurotoxin only addresses the muscle component. If the concern is softness, fullness, or loose skin rather than muscular development, this treatment won't change it — and a different approach is needed. That distinction is made at consultation.

Not appropriate if you train seriously, play a sport requiring pressing or throwing strength, or do physical work involving repeated lifting or lowering.
